Release of Student Application Form for Admission (The soft copy is available and can be downloaded at the Official FB Page of the school and Guidance and Counseling Office. The hard copy is also available at the Guidance and Counseling Office).

  1. ELIGIBLE APPLICANTS

 

  • Graduating Grade 12 Senior High School students/applicants from public and/or private secondary schools duly recognized by the government.
  • Students/applicants who are Senior High School graduates before the school year 2020 – 2021.
  • Transferee students/applicants from any public and/or private Higher Education Institutions (HEI’s).

 

  • GUIDELINES AND PROCESS FOR THE TEST – FREE ADMISSION OF INCOMING FIRST YEAR COLLEGE STUDENTS AND TRANSFEREES

 

  1. The DEBESMSCAT College Entrance Test (DECET) is postponed for this A.Y 2021 – 2022 due to the CoViD–19 pandemics and the existing restrictions implemented by the Department of Health (DOH) and Inter–Agency Task Force (IATF) along with physical mobility, any large crowd gatherings, and face to face transactions.
  2. The admission or evaluation for college eligibility will be based on the applicants’ General Weighted Average (GWA) from Grade 9–11 stated in the certification of eligibility to enter college signed by their Principal. Applicants who do not meet the grade requirement set by each respective college will be on a conditional status.
  3. The online and offline application will start on March 01, 2021 until May 31, 2021. Late application and submission of requirements will not be accommodated.
  4. The Student Application Form for Admission is available and can be downloaded at the Official FB page of the school and Guidance and Counseling Office. The links for the forms will be sent to those who will request. Hard copy of the form is also available in the Guidance and Counseling Office. (The application form is strictly not for sale).
  5. There are three (3) available options for the application process and submission of requirements.

OPTION A

Note: To proceed with the application, you need to use the app Adobe Acrobat on your preferred device (PC, phone, tablet, etc.). You are advised to download the app first before proceeding. Other applications have a “fill and sign” feature and can also be one of your choices to fill out the form.

  1. Download the appropriate fillable forms (Student Application Form for Admission, List of Curricular Offerings and Principal Certification Form) on the link below:

Form 1.A – for Graduating Grade 12 Senior High School applicants and Senior High School graduates before the school year 2020–2021.

Form 2 – Principal Certification Form

Form 1.B – for transferee applicants.

  1. Fill out all the necessary fields. Required fields are highlighted in red.
  2. Check the second page of the form for the list of curricular offerings/academic courses and the corresponding codes.
  3. Click on the image placeholder at the upper right-hand corner of the form and upload your 2x2 ID picture.

The 2x2 ID picture should have a white background with a name tag.

  1. After filling out the form, click on the pen button/icon that appears on the lower right-hand corner of your screen/phone and select “fill and sign” (Note: other applications may have a different method for accessing the fill and sign feature). This will give you an option to create your signature by writing on your screen using your finger or select an existing signature (if you have one already). Do this for the applicant, parent, or guardian to sign near the end of the form.
  1. Print the Form 2 Principal Certification Form (third page of your application form) on a long sized bond paper (8’x13’).
  2. Have the School Registrar or other authorized personnel (of the High School you are enrolled in) accomplish the form (including his/her signature) and secure the Principals' signature. For the applicants who are Senior High School graduates before the school year 2020-2021, you are not required to provide the Principal Certification Form instead, photocopy of your Form 137 and Form 138 in Grade 12 are needed. For transferee applicants, secure a transcript of records (TOR) and certification from the school registrar of your former college/university indicating the period of your stay in the said institution.
  3. Scan the accomplished forms and other needed documents and secure the softcopy (in JPEG, PNG, or PDF format).

 

  • For Graduating Grade 12 Senior HS Applicants – Student Application Form for Admission (Form 1.A) and Principal Certification Form (Form 2).
  • For Senior HS Graduates before the School Year 2020–2021 – Student Application Form for Admission (Form 1.A) and scanned copy of Form 137 and Form 138 in Grade 12.
  • For Transferee Applicants – Student Application Form for Admission (Form 1.B) and scanned copy transcript of records (TOR) and certification from the school registrar of your former college/university.
  1. Open the Application Submission Form on the link below and upload the soft copy of accomplished forms. https://forms.gle/Ux61vV4QWdWMNAM38
  2. Fill out the fields in the form and upload your accomplished forms. (See No.8) You can find the upload buttons at the end of the form.
  3. Wait for the confirmation message via e-mail by the Guidance and Counseling Office through the email address used in the submission of forms in No. 9. (Don’t forget to include your e-mail address in the application form.)
  4. Visit the FB page of the respective college for the schedule (of the interview, diagnostic exam, microteaching*, etc.) and additional information.

  1. Print it out and accomplish the form completely and accurately.
  1. Check the second page of the form for the list of curricular offerings/academic courses and the corresponding codes.
  1. Attach a 2x2 ID picture on the image placeholder at the upper right-hand corner of the application form.

The 2x2 ID picture should have a white background with a name tag.

  1. Print the Form 2 Principal Certification Form (third page of your application form) on a long sized bond paper (8’x13’).

 

  1. Have the School Registrar or other authorized personnel (of the High School you are enrolled in) accomplish the form (including his/her signature) and secure the Principals' signature.   1. Check the second page of the form for the list of curricular offerings/academic courses and the corresponding codes.

  1. Place and seal the accomplished forms and other needed documents in a long brown envelope and long plastic envelope.
  • For Graduating Grade 12 Senior HS Applicants – Student Application Form for Admission (Form 1.A) and Principal Certification Form (Form 2).
  • For Senior HS Graduates before the School Year 2020–2021 – Student Application Form for Admission (Form 1.A) and photocopy of Form 137 and Form 138 in Grade 12.
  • For Transferee Applicants – Student Application Form for Admission (Form 1.B) and photocopy of transcript of records (TOR) and certification from the school registrar of your former college/university.
  1. Hand - in your submissions to the Guidance and Counseling Office in person or via a representative provided that they comply the health protocols set by the DOH and IATF. Submission of application forms and other documents in cluster or group is also possible. For applicants outside the province, you may send them, via your preferred courier, to:

  2. Photocopied PSA birth certificate (bring original for verification)
  3. Original copy of police clearance
  4. Original copy of certification of good moral character issued by the Principal or Guidance Counselor of the school last attended
  5. Photocopied PSA marriage certificate if married (bring original for verification)
  6. Medical certificate issued by the College Medical Officer
  7. Two copies of recent 2x2 photo with white background
B. Transfer Student
  1. Transcript of records (TOR)/ Copy of grades
  2. Original copy of police clearance
  3. Original copy of certification of honorable dismissal/certification of good moral character
  4. Photocopied PSA birth certificate (bring original for verification)
  5. Photocopied PSA marriage certificate if married (bring original for verification)
  6. Medical certificate issued by the College Medical Officer

  3. Photocopied NSO Birth Certificate

K. Doctoral Students

  1. Original copy of TOR w/ SO in the Master’s Degree for graduate in private school / CHED supervised HEI and with BOT Resolution For graduate in SUC (copy valid for DEBESMSCAT)
  2. 2 pcs 2x 2 ID picture w/ white background
  3. Photocopied NSO Birth Certificate
  4. Photocopied NSO Marriage Certificate (for married)
